POSITION OVERVIEW

 

As Certified Nursing Assistant (CNA), you will be at the forefront of the company working in a rewarding, challenging and dynamic environment as the go-to person for our clients. In this important role, you will  be working with a great team of professionals and be responsible for assisting clients in either the home health or hospice lines of services who are chronically ill, recovering from an illness, injury or who are terminally ill.  You will assist and support  home health and hospice clients to continue to live a quality of life in their own homes and communities.

 

Successful candidates will travel to client’s houses and assist the clients with the tasks of daily living as outlined in the written plan of care established by the clinical team.

  • Task oriented- Must be able to follow directions per plan of care,  managing multiple tasks such as providing assistance in client’s home cleanliness/safety, providing standby or full assistance regarding, making beds, cooking meals, bathing , dressing/undressing clients, observing/reporting changes in the condition of the client, tracking clients and strong active listening skills.
  • Results driven individual that can meet or exceed client's needs in accordance with professional standards of practice through center policies and procedures, and federal, state and local guidelines.
  • People oriented individual- ability to interact and manage a diversity of clients with positivity and are open and receptive to coaching and feedback.
  • Collaborative fast paced environment, working independently and with teammates. Must be confident to be self-sufficient and utilize resources to serve client’s needs.
  • Attend bi-weekly team conference meetings
  • Maintains confidentiality and does not discuss client or client’s information outside of the company
  • Must have available reliable transportation to and from client’s homes.

WHAT YOU WILL DO

  • Provide a positive, professional, empathetic and client-focused atmosphere
  • Perform clerical duties such as filing, data entry, operating multiple computer systems
  • Understand and maintain the rules of HIPAA for client confidentiality
  • Audit files for quality and completeness
  • Assist with processing paperwork
  • Support clients with web-based applications
  • Communicate effectively via phone, E-Mail, and in person with Program Staff
  • Assist with payroll review processes and questions
  • Maintain a high attention to detail and the ability to think creatively to solve problems
  • Communicate appropriately with supervisor on reports and administrative updates
  • Be a team player- ability to work in teams, are open and receptive to coaching and feedback, and provide feedback to others
  • Perform general office duties and assisting where needed

QUALIFICATIONS

  • 1 year administrative/quality assurance experience
  • Computer proficiency to include Microsoft suite, specifically Outlook and Excel and experience with Customer Relationship Management software preferred
  • Basic math skills
  • Must be able to successfully pass a background screening
  • Experience in Health Care field preferred; but not required
  • High School Diploma required
  • College education preferred, degree not required

WHAT YOU WILL DO

 

  • Provide services in a positive, professional, empathetic and client-focused manner
  • Perform home visits for program enrollment and ongoing monitoring
  • Complete appropriate and detailed documentation and correspondence for participants, including all required forms
  • Maintain effective verbal communication
  • Ensure a positive, collaborative and cohesive team environment
  • Attend educational meetings, training sessions, and other meetings as needed or required by the program
  • Ensure quality customer service

 

QUALIFICATIONS

  • Must meet one of the following educational criteria:
    • Bachelor’s degree with two years’ experience working with individuals with disability or the elderly – or –
    • Master’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Gerontology or Sociology with one year experience working with individual with disabilities or the elderly– or –
    • Registered Nurse (RN) or an Associate degree and three years’ experience working with individuals with disabilities or the elderly
  • District residency required
  • Effective verbal and written communication skills
  • Computer proficient specifically with Outlook and Excel
  • Ability to travel throughout the District (if using own vehicle must have valid insurance)
  • Must be able to successfully pass a background screening

CSCT Behavior Specialist – Cold Springs Elementary Structured Learning Program

 

Consumer Direct Care Network Behavioral Health (formerly known as Full Circle Counseling Solutions) is seeking a full-time Behavioral Specialist to provide school-based (CSCT) mental health behavioral support services at a Missoula elementary structured learning program under a recent team expansion. 

 

The ideal candidate for this position will possess experience and skills needed for working with children with behavioral and emotional challenges. Candidates for this position will require creative thinking, flexibility, problem-solving, strong teamwork, de-escalation and crisis response skills, a passion for alternative learning environments, and an ability to engage with parents, teachers, school administrators and colleagues in a positive and goal-oriented atmosphere. 

 

This position will provide individual and group-based interventions focused on the development of social skills interactions, managing emotional and behavioral challenges, teaching coping skills, and integrating interventions focused on positive behavioral support.  Bachelor’s degree in related field required.

CSCT Therapist – Cold Springs Elementary Structured Learning Program

 

 

Consumer Direct Care Network Behavioral Health (formerly known as Full Circle Counseling Solutions) is seeking a full-time mental health professional (LCPC, LCSW or in-training Master’s level candidates already possessing a Montana candidate license).  The successful candidate will provide school-based (CSCT) mental health services in a Missoula elementary school structured learning program. This position acts as the clinical lead for a two-person intervention team. 

 

The ideal candidate for this position will have experience working with youth with emotional or behavioral challenges, diagnostic and treatment planning skills, de-escalation and crisis response skills, and a demonstrated ability to work collaboratively with parents, teachers, school and community members in a positive and goal-oriented atmosphere.  Demonstrated knowledge of social thinking models and teaching social skills, life skills, conducting individual and family therapy sessions, facilitating groups, participating in collaborative functional behavior assessments, and working in a team-based environment is preferred. 

 

  • This position will require creative thinking, flexibility, problem-solving abilities, ability to utilize time efficiently, and a passion for working with challenging youth in a school setting. 
  • Strong preference will be given to licensed clinicians. 
  • In-training clinicians must hold a current LCPC or LCSW candidate license from the Montana Board of Behavioral Health.

As a Hospice Nurse Case Manager, you will be at the forefront of the company working in a rewarding, challenging and dynamic environment.  Hospice Nurse Case Manager plans, organizes and directs hospice care. In this important role, you will be responsible for supporting patients who are terminally ill and providing end of life care to patients and their families so they can continue to live fully.  Consumer Direct Care Network Montana-Home Health & Hospice mission is to support patients and families through the last stages of life with dignity, grace, strength, and comfort.

Patient Care-

Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.

 

Provides a complete physical assessment and history of current and previous illness(es).  Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.

 

Assess and evaluates patient’s status by: writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs and revising plan of care as necessary.

 

Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.

 

Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

Counsels the patient and family in meeting nursing and related needs.

 

Provides health care instructions to the patient as appropriate per assessment and plan.

Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.  Records pain/symptom management changes/outcomes as appropriate.

 

Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.

 

Communicates with community health related persons to coordinate the care plan.

 

Teaches the patient and family/caregiver self-care techniques as appropriate.  Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.  Works in concert with the interdisciplinary group.

 

Provides and maintains a safe environment for the patient.

 

Assists the patient and family/caregiver and other team members in providing continuity of care.

 

Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

 

Attends interdisciplinary group meetings.

 

Additional Duties

 

Participates in on-call duties as defined by the on-call policy.

 

Ensures that arrangements for equipment and other necessary items and services
are available.

 

Supervises ancillary personnel and delegates responsibilities when required.

 

Actively participates in quality assessment performance improvement teams and activities.

 

Requirements to apply:

Paid on a per visit basis, the Field Coordinator will complete enrollment visits in our Participants homes. This includes assisting filling out paper work and training on how to submit time to pay their workers. The Field coordinators also have administrative duties including setting up appointments, documentation and submitting paperwork. They are “telecommuters” and work from their home office. In the Consumer Direct Care Network employees are committed, dependable, professional and accountable. We value integrity, honesty and respect for others. RESPONSIBILITES INCLUDE

 

  • Act as a liaison between Participants and the Consumer Direct Office
  • Complete meetings in participants’ homes, enrolling them into Participant Directed Option (PDO) services
  • Set up new participants in the Consumer Direct data base
  • Complete documentation of enrollment meetings and promptly return paperwork to the home office
  • Gain knowledge and stay informed of changes regarding Participant Directed Option services

The Field Coordinator will project a positive image and will be the face of CDCN with the Consumers and general public. The Coordinator will serve as the local point of contact for the branch office to utilize for field assignments.

  • Per Visit Payment: The Coordinator will be paid on a per visit basis as follows:

- $90.00 for less than 50 mile RT visit

- $110.00 for a 50 - 100 mile RT visit

- $120.00 for a greater than 100 mile RT visit

 

This payment is all inclusive of the visit requirements including scheduling and assembling enrollment packets, completing the visit, auditing the completeness of the enrollment papers, documenting, and forwarding information to the branch office. CDCN is not providing a guarantee of visit volume in this agreement.

 

  • Administrative/Training Hours: The Coordinator will be reimbursed at a rate of $12.00 per hour for time spent receiving training.

 

  • Transportation & Vehicle Insurance: Coordinator will need to have reliable transportation for business travel in connection with work for CDCN. Coordinator agrees to maintain liability insurance with a minimum coverage of $300,000.00 per occurrence.

 

QUALIFICATIONS

 

  • Must be able to successfully pass level 2 background check.
  • Must have reliable transportation, car insurance and valid driver license for travel in the surrounding.
  • Bilingual English/Spanish preferred, but not required.

POSITION OVERVIEW

 

Paid on a per visit basis, the Field Coordinator will complete enrollment visits in our Participants homes. This includes assisting filling out paper work and training on how to submit time to pay their workers. The Field coordinators also have administrative duties including setting up appointments, documentation and submitting paperwork. They are “telecommuters” and work from their home office. In the Consumer Direct Care Network employees are committed, dependable, professional and accountable. We value integrity, honesty and respect for others. RESPONSIBILITES INCLUDE

 

  • Act as a liaison between Participants and the Consumer Direct Office
  • Complete meetings in participants’ homes, enrolling them into Participant Directed Option (PDO) services
  • Set up new participants in the Consumer Direct data base
  • Complete documentation of enrollment meetings and promptly return paperwork to the home office
  • Gain knowledge and stay informed of changes regarding Participant Directed Option services

The Field Coordinator will project a positive image and will be the face of CDCN with the Consumers and general public. The Coordinator will serve as the local point of contact for the branch office to utilize for field assignments.

  • Per Visit Payment: The Coordinator will be paid on a per visit basis as follows:

- $90.00 for less than 50 mile RT visit

- $110.00 for a 50 - 100 mile RT visit

- $120.00 for a greater than 100 mile RT visit

 

This payment is all inclusive of the visit requirements including scheduling and assembling enrollment packets, completing the visit, auditing the completeness of the enrollment papers, documenting, and forwarding information to the branch office. CDCN is not providing a guarantee of visit volume in this agreement.

 

  • Administrative/Training Hours: The Coordinator will be reimbursed at a rate of $12.00 per hour for time spent receiving training.

 

  • Transportation & Vehicle Insurance: Coordinator will need to have reliable transportation for business travel in connection with work for CDCN. Coordinator agrees to maintain liability insurance with a minimum coverage of $300,000.00 per occurrence.

RESPONSIBILITES INCLUDE, BUT ARE NOT LIMITED TO

  • Act as a liaison between clients, employees and Consumer Direct.
  • Coordinate, schedule and complete in person meeting for the Self Directed Program.
  • Route all paperwork, including payroll information, in a timely manner for processing.
  • Abide by and maintain all confidentiality requirements in communications and information regarding clients, employees and business practices.
  • Assist in maintaining compliance for all program, state and federal rules and forms.
  • Attend meetings, trainings and conferences when required.

 

QUALIFICATIONS

  • At least a high School Diploma with at least 2 to 3 years of experience in the field of social and human services or more.
  • Must be able to successfully pass a background check.
  • Must have reliable transportation, car insurance and valid driver license for travel.
  • Must be very reliable, detail oriented and have exceptional abilities to work well independently in a remote team environment.
  • Must possess an ability to network, capture and maintain positive and ongoing professional relationships with internal and external teams.
  • Preferred prior experience in Medicaid and Medicare services in the state of Michigan but not required.

  • Assesses patient condition, establishes goals, treatments and care plan in conjunction with the physician, patient/family
  • Ensures that appropriate treatment is performed in interdisciplinary fashion, consulting with physicians about impact of physical therapy factors on the patient's health status
  • Documents findings, treatments and other patient information in a timely manner in the electronic medical record
  • Educates patient/family on appropriate care and use of various therapies/exercises
  • Serves as information/education resource for patients, families and staff
  • Provides on-going education to the Physical Therapy staff as needed
  • Supervise those treatment delegated to Physical Therapy Assistants (PTAs) and nurses’ aides
  • Maintains patient confidentiality
  • Must be knowledgeable in physical therapy principles, standards and applications

 

QUALIFICATIONS

  • Current State of Montana Physical Therapy license
  • Experience in home health preferred, but not necessary (Willing to train the right candidate)
  • 1 year Physical Therapy experience in an acute care and/or rehabilitative setting
  • Must be able to successfully pass a background screening

Successful candidates could manage up to 5 members per day, give or take depending on the case load and will travel to the member’s residence to provide in home services. These services are typically directed by a state plan to adequately assist and provide outstanding and personalized day-to-day care for each member. The Core PCA position includes both in the field and office based work to handle member’s needs and administrative duties.

The Consumer Direct Behavioral Health Developmental Disabilities Program is hiring for a Board Certified Behavior Analyst.

 

Position Overview

 

The BCBA is responsible overseeing client’s services. This person will work with members of an interdisciplinary team to provide effective behavior support. The BCBA will work with clients with varying disabilities, their families, schools, and community providers. Responsibilities of the BCBA will include the following:

 

  • Conduct developmental and criterion-referenced assessments
  • Design appropriate, function-based interventions
  • Assist program with data collection to determine effectiveness of interventions
  • May participate in staff training related to implementation of plans
  • Provide supervision and training to Family Support Specialists and Children’s Autism Trainers


Qualifications:

  • Certification with the Behavior Analyst Certification Board or eligibility to sit for the BACB exam within six months of hire
  • Experience working with children with Autism Spectrum Disorder
  • Experience with supervision and oversight of staff and program development
  • Great interpersonal skills and exceptional time management skills

As a Core PCA, you will be at the forefront of the company working in a rewarding, challenging, and dynamic environment as the go-to person for our members! In this important role, you'll be responsible for supporting individuals of all ages with disabilities to high risk members in need of immediate assistance. You will provide care for those who choose to maintain a quality of life in their own homes and communities. Successful candidates could manage up to 5 members per day, give or take depending on the case load and will travel to the member’s residence to provide in home services. These services are typically directed by a state plan to adequately assist and provide outstanding and personalized day-to-day care for each member. The Core PCA position includes both in the field and office based work guaranteeing you 40hrs a week.

  • Competency in managing multiple tasks, such as furnishing assistance in maintaining cleanliness / safety in the home of the member. Providing standby or full assistance regarding: making beds, cooking meals, bathing, toileting, dressing / undressing members, observing and tracking any changes in the condition of the member, while utilizing strong active-listening skills.
  • Results driven individual that can meet or exceed the needs of the member in accordance with professional standards of practice through center policies and procedures; including federal, state, and local guidelines.
  • Optimistic and social personality capable of receiving progressive direction and feedback.
  • Strong capacity to interact and manage a multitude of diverse members.
    Collaborative fast paced environment with requirements of working both independently and with teammates at times. Must be confident and self-sufficient to utilize resources which will best serve the needs of the member.
  • Ability to interact and evaluate circumstances and subsequently offer solutions in situations when members may become distressed.
  • Implement written company procedures and guidelines, resource tools, and effectively facilitate success in conflict.
  • Maintains confidentiality and does not discuss identifying information of any member outside of the company.
  • Administrative duties to include answering phones, barcoding paperwork and filing documentation appropriately.

 

 

QUALIFICATIONS

 

  • PCA Training offered
  • Must be willing to take a TB test provided by company if you already do not have one
  • Pass a physical test to include safely lifting 75 pounds
  • Must be able to successfully pass a background screening
