POSITION OVERVIEW

 

As a Core PCA, you will be at the forefront of the company working in a rewarding, challenging, and dynamic environment as the go-to person for our members! In this important role, you'll be responsible for supporting individuals of all ages with disabilities to high risk members in need of immediate assistance. You will provide care for those who choose to maintain a quality of life in their own homes and communities. Successful candidates could manage up to 5 members per day, give or take depending on the case load and will travel to the member’s residence to provide in home services. These services are typically directed by a state plan to adequately assist and provide outstanding and personalized day-to-day care for each member. The Core PCA position includes both in the field and office based work to handle member’s needs and administrative duties.

WHAT YOU WILL DO

 

  • Provide a caring, compassionate, empathetic, and professional in a client-focused manner
  • Competency in managing multiple tasks, such as furnishing assistance in maintaining cleanliness / safety in the home of the member. Providing standby or full assistance regarding: making beds, cooking meals, bathing, toileting, dressing / undressing members, observing and tracking any changes in the condition of the member, while utilizing strong active-listening skills.
  • Results driven individual that can meet or exceed the needs of the member in accordance with professional standards of practice through center policies and procedures; including federal, state, and local guidelines.
  • Optimistic and social personality capable of receiving progressive direction and feedback.
  • Strong capacity to interact and manage a multitude of diverse members.
  • Collaborative fast paced environment with requirements of working both independently and with teammates at times. Must be confident and self-sufficient to utilize resources which will best serve the needs of the member.
  • Ability to interact and evaluate circumstances and subsequently offer solutions in situations when members may become distressed.
  • Implement written company procedures and guidelines, resource tools, and effectively facilitate success in conflict.
  • Maintains confidentiality and does not discuss identifying information of any member outside of the company.
  • Administrative duties to include answering phones, barcoding paperwork and filing documentation appropriately.

 

 

 

QUALIFICATIONS

 

  • PCA Training offered
  • Must be willing to take a TB test provided by company if you already do not have one
  • Pass a physical test to include safely lifting 75 pounds
  • Must have a valid driver’s license and appropriate vehicle insurance as some travel is required
  • Current CPR and First Aid Certificates (will train)
  • Must be able to successfully pass a background screening

POSITION OVERVIEW

 

Consumer Direct Care Network has a rewarding career opportunity available as a Compliance Coordinator located in our Sunrise, FL office. The ideal candidate will have a “big picture” view and carry a high attention to detail to catch any mistakes that are vital to operating compliantly. The primary duty of the Compliance Coordinator is to ensure that all aspects of our Florida operation are compliant with state and contractual guidelines regarding the Managed Care Organization. The Compliance Coordinator will ensure Nurse Licenses, participant documents and company contracts are all compliant, to name a few. It is important that our services operate issue free within both HIPAA and state regulations. Equally important in this position, the Coordinator will help prepare and provide documents as requested for audits and state agencies.

WHAT YOU WILL DO

 

  • Ensure compliance in all aspects of Consumer Direct Care Network Florida
  • Monitor and Report of contractual and state requirements utilizing spreadsheets and web based applications
  • Monitor and complete Fraud, Waste and Abuse (FWA) reporting
  • Review casefiles to ensure requirements are met for restarts of participants and Direct Service Workers
  • Contact Representatives and Direct Service Workers and monitor follow-up of background check renewals
  • Monitor company software to ensure compliance of participant, representative and direct service worker documents
  • Review timesheets and other documents requested by the Managed Care Organization (MCO) or other state entities prior to uploading or e-mailing to MCO
  • Process documentation for terminations (monthly billing and vendor payments)
  • Monitor nursing licenses to ensure compliance
  • Ensure Administrator and Coordinator portions of referral tracker are complete such as dates and signatures
  • Assist with maintaining company proficiency trackers
  • Monitor compliance of E-Verify documents and renewals
  • Train internal staff on policies and procedures
  • Other duties may be assigned as needed

 

QUALIFICATIONS

  • Bilingual in English/Spanish both oral and written communications
  • Strong experience with Microsoft office and willingness to learn other computer applications
  • 2 years’ direct experience with Quality Assurance and or Compliance
  • Experience writing Standard Operating Procedures
  • Training of staff preferred
  • Must be able to successfully pass a level II background screening

CSCT Clinician – Middle School Structured Learning Program

 

Consumer Direct Care Network Behavioral Health (formerly Full Circle Counseling Solutions) is seeking a full-time mental health professional (LCPC, LCSW or Master’s level, license-eligible) to provide school-based (CSCT) mental health services to a Missoula middle school structured learning program. The ideal candidate for this position will have experience working with adolescents with emotional or behavioral challenges, diagnostic and treatment planning skills, de-escalation and crisis response skills, and a demonstrated ability to work collaboratively with parents, teachers, school and community members in a positive and goal-oriented atmosphere. 

 

Demonstrated knowledge of social thinking models and teaching social skills, facilitating groups, and working in a team-based environment is preferred. This position will require creative thinking, flexibility, problem-solving abilities, and a passion for alternative learning environments.  In-training practitioners must apply for and receive provisional licensing status within the first 180 days of employment. Strong preference will be given to licensed clinicians. 

 

This is a full-time, year-round position with competitive salary and benefits. This position will remain open until filled, anticipated start date will be mid-August.

POSITION OVERVIEW

 

Consumer Direct of Florida is seeking a people-oriented Time Sheet Auditor to work in a dynamic and fast paced environment located in our Sunrise office. In this important role you will assist the staff in many office clerical type capacities from taking high volume in bound phone calls, greeting walk-in customers and operating computer systems to supporting business operations in an administrative type capacity.

 

The Timesheet Auditor is responsible for processing and coordinating timesheets as well as performing general office duties as needed.  This person is a reflection of Consumer Direct and must be positive, professional and gracious in manner, always representing the company with a positive attitude.

WHAT YOU WILL DO

  • Provide a positive, professional, empathetic and customer-focused manner
  • Sitting for long periods of time managing multiple tasks such as strong active listening while typing and conversing with the customers in addition to filing, data entry, operating multiple computer systems and routing inbound phone calls. 
  • Review timesheets for accuracy, ensure timeliness is maintained and process according to agency standards
  • Work with Excel Spreadsheets to Prevent and Detect Fraudulent Timesheets
  • Perform daily follow-up phone calls with consumers and caregivers regarding inaccurate timesheets and answering any questions
  • Work various reports to ensure that all timesheets are received and correct, based off of authorizations.
  • Inform Program Coordinators of any important changes noted by consumer or caregiver on timesheets
  • Answer incoming telephone calls, responding to basic questions and/or forwarding to appropriate personnel when required
  • Attend meetings and trainings as required
  • Maintain noting in CRM on a daily basis to ensure organization and communication in the errors on the timesheets.
  • Assist with administrative duties including answering the phones, sorting mail, processing Member/Caregiver packets and other duties as required

 

QUALIFICATIONS

  • 1 years administrative/clerical experience 
  • Bilingual in English/Spanish required
  • Computer proficiency to include Outlook and Excel
  • Basic math skills
  • Reliable transportation and valid driver’s license required.
  • Must be able to successfully pass a background screening.

POSITION OVERVIEW

 

Consumer Direct Management Solutions is seeking a self-starting; people oriented Training Specialist to join our operations team located in Missoula, MT.  The Training Specialist will be tasked with training Consumer Direct employees on companywide programs. These programs range from known software such as Microsoft applications to internal company software such as Docuware and CRM and everything in between. As the company trainer, you will be engaging 1-on-1 training sessions to groups as large as 100 people at times. The Training Specialist must be comfortable with instructing people with different learning styles and present in different forms of communication from online to in person presentations. This position will require the individual to have a high level of verbal and written communication skills and strong organizational/project management skills with the ability to work independently and meet deadlines.

POSITION OVERVIEW

 

Consumer Direct Care Network, a company dedicated to providing the highest quality of in-home health care services, is eager to fill a Registered Nurse role in Allen, TX.  The RN is responsible for ensuring that patient care is coordinated and managed accordingly and that services are delegated appropriately by clinical personnel. The RN will also be responsible for the training of new Direct Care Staff.

WHAT YOU WILL DO

  • Understand Medicare Conditions of Participations for home health and ensure compliance of home health program
  • Delegate and supervisor Direct Care Staff
  • Train Direct Support Staff to provide home health services accordingly
  • Collaborate with home health team to address trends that affect the agency’s outcome and process measures
  • Receive case referrals and review patient’s clinical diagnosis, medications, procedures and clinical course in order to determine home care needs
  • Assign appropriate clinicians and therapists to case, and provide support in establishing and developing plan of care
  • Must be available during operating hours to assist clinicians and therapists as appropriate
  • Conduct quarterly record reviews and communicate findings and recommendations to Clinical Director and appropriate personnel
  • Assist in the screening, interviewing and orientation process of new personnel
  • Assist State Director with the planning and implementation of in-service and continuing education programs
  • Assist in the formulation and implementation of policies and procedures, as well as strategic goals and objectives
  • Lead biweekly case conference meetings with home health team members and staff meetings as needed
  • Participate in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system

 

QUALIFICATIONS

  • Registered Nurse with current licensure to practice in the state
  • Graduate of an accredited school of nursing. Bachelor’s degree preferred

  • Understand Medicare Conditions of Participations for home health and ensure compliance of home health program
  • Manage and oversee two RNs
  • Train Direct Support Staff to provide home health services accordingly

Consumer Direct Management Solutions is seeking a Billing Analyst to join our Claims Department at our Missoula location. In this important role, you will be required to sit for long periods of time working with multiple computer systems processing payments from Medicaid so that we can receive funds for the care that we provide to our members. This role requires an enormous amount of attention to detail and patience to work on complex tasks for long periods of time. As a Billing Analyst you will be tasked with additional duties such as the investigation, correcting and resubmission of claims along with cross-department collaboration to ensure the proper processing of claims.

  • Prepare claims and/or invoices for all clients serviced by assigned program(s)/state(s).
  • Correct claims and/or invoices and resubmit as necessary. This task will require coordination with other Billing Team members, Branch Office staff, management and payers (i.e., State Medicaid, insurance companies, city/county agencies, public and private companies, and individuals).
  • Enter claim adjustments according to CDMS policy and procedure.
  • Meet timely filing requirements as set forth by our trading partners or payers.
  • Record denial codes, research unpaid claims, correct and re-submit claims for payment. Document this work in anticipation of having to prepare and submit an appeal for denied claims.
  • Ensure that electronic and paper claims are received by the trading partner or payer. Correct problems as necessary.
  • Attend weekly briefings and present progress reports regarding program specific Accounts Receivable.
  • Provide support for special projects within the Billing Department.
  • Identify and communicate billing and/or collection problems to Billing Team Lead.
  • Demonstrates full understanding of accounting principles, how transactions flow to their final destination and how this affects the end result on the financial statements.
  • Other duties as assigned by Billing Team Lead and Claims Manager.

 

QUALIFICATIONS

  • High School Diploma or Associate degree preferred
  • 1-3 years of experience in business/accounting required
  • Strong organizational/project management skills required
  • Effective interpersonal communication skills required
  • Strong knowledge of MS Office Suite required
  • Knowledge of Access or SQL database software and functions preferred

As a Social Worker you will provide supportive and medical social services to patients recovering from illness/surgeries and hospice patients and their families in accordance with their plan of care. The Social Worker will assume responsibility for the development, implementation, treatment and management of psychosocial services provided to home health and hospice patients and their families in the home care or residential setting.

  • Participate in the development of the individualized plan of care, involving the patient and family
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment

  • Carry out social evaluations including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings
  • Counsel patient and family/caregivers as needed regarding stress and other identified coping difficulties
  • Provide crisis intervention when necessary
  • Assess for and educate interdisciplinary group on any special needs related to the culture of the patient and family to include communication, role of family, space, and any special traditions or taboos
  • Educate patients and families on and assists in preparation of advanced directives
  • Provides information and serves as a liaison to patients and families/caregivers and community resources
  • Maintain and develop contracts with public and private agencies as resources for patient and personnel
  • Attend hospice interdisciplinary group meetings and home health care conferences as scheduled
  • Coordinate the delivery of grief counseling needs and services
  • Assist team members in understanding significant social and emotional factors related to health problems and death/dying issues
  • Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
  • Provide counseling support as needed to home health and/or hospice personnel coping with work related grief
  • Provide bereavement information and referral services to callers from the community
  • Other duties may be delegated by the Clinical Supervisor

 

QUALIFICATIONS

  • Master of Social Work accredited by the Council on Social Work Education
  • Experience in field of home health and hospice preferred, but not required
  • Understanding of hospice philosophy and issues of death/dying
  • Understanding of home health services
  • Must have current CPR certification
  • Excellent writing skills with high attention to detail
  • Good verbal, written and organization skills

  • Results driven individual that can meet or exceed the needs of the member in accordance with professional standards of practice through center policies and procedures; including federal, state, and local guidelines.
  • Optimistic and social personality capable of receiving progressive direction and feedback.
  • Strong capacity to interact and manage a multitude of diverse members.
    Collaborative fast paced environment with requirements of working both independently and with teammates at times. Must be confident and self-sufficient to utilize resources which will best serve the needs of the member.
  • Ability to interact and evaluate circumstances and subsequently offer solutions in situations when members may become distressed.

POSITION OVERVIEW

 

As a Core PCA, you will be at the forefront of the company working in a rewarding, challenging, and dynamic environment as the go-to person for our members! In this important role, you'll be responsible for supporting individuals of all ages with disabilities to high risk members in need of immediate assistance. You will provide care for those who choose to maintain a quality of life in their own homes and communities. Successful candidates could manage up to 5 members per day, give or take depending on the case load and will travel to the member’s residence to provide in home services. These services are typically directed by a state plan to adequately assist and provide outstanding and personalized day-to-day care for each member. The Core PCA position includes both in the field and office based work guaranteeing you 40hrs a week.

WHAT YOU WILL DO

 

  • Provide a caring, compassionate, empathetic, and professional in a client-focused manner
  • Competency in managing multiple tasks, such as furnishing assistance in maintaining cleanliness / safety in the home of the member. Providing standby or full assistance regarding: making beds, cooking meals, bathing, toileting, dressing / undressing members, observing and tracking any changes in the condition of the member, while utilizing strong active-listening skills.
  • Results driven individual that can meet or exceed the needs of the member in accordance with professional standards of practice through center policies and procedures; including federal, state, and local guidelines.
  • Optimistic and social personality capable of receiving progressive direction and feedback.
  • Strong capacity to interact and manage a multitude of diverse members.
    Collaborative fast paced environment with requirements of working both independently and with teammates at times. Must be confident and self-sufficient to utilize resources which will best serve the needs of the member.
  • Ability to interact and evaluate circumstances and subsequently offer solutions in situations when members may become distressed.
  • Implement written company procedures and guidelines, resource tools, and effectively facilitate success in conflict.
  • Maintains confidentiality and does not discuss identifying information of any member outside of the company.
  • Administrative duties to include answering phones, barcoding paperwork and filing documentation appropriately.

 

QUALIFICATIONS

 

  • PCA Training offered
  • Must be willing to take a TB test provided by company if you already do not have one
  • Pass a physical test to include safely lifting 75 pounds
  • Must be able to successfully pass a background screening
