    Consumer Direct Care Network is seeking a people-oriented Case Manager (Support Broker Level I) to work in a dynamic yet fun and collaborative environment. In this important role you will assist individuals managing their services through a self-directed Medicaid program. Successful candidates will emphasize their ability to support individuals with disabilities and the elderly by promoting prompt, respectful, and professional service.

    • Provide services in a positive, professional, empathetic and client-focused manner
    • Perform monthly contacts and ongoing monitoring to ensure compliance
    • Complete appropriate and detailed documentation and correspondence for participants, including all required forms
    • Demonstrate effective verbal communication
    • Maintain confidentiality regarding all communication/information related to clients, attendants, department contacts, and all business relations
    • Promote a positive, collaborative, and cohesive team environment
    • Attend educational meetings, training sessions, and other meetings as needed or required by the program
    • Ensure that quality customer service is provided


    • Education and Experience: 
      Must meet one of the following educational criteria:

      • Some post-secondary education required
      • Minimum experience of 1 year working with individuals with disabilities and/or the elderly
    • Effective verbal and written communication skills
    • Computer proficient specifically with Outlook and Excel
    • Must be able to successfully pass a background screening
    • Must be able to frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
