    Paid on a per visit basis, the Field Coordinator will complete enrollment visits in our Participants homes. This includes assisting filling out paper work and training on how to submit time to pay their workers. The Field coordinators also have administrative duties including setting up appointments, documentation and submitting paperwork. They are “telecommuters” and work from their home office.

    •  Act as a liaison between participants and the CDCN office
    • Complete meetings in participants’ homes, enrolling them into Participant Directed Option (PDO) services
    • Set up new participants in the CDCN data base
    • Complete documentation of enrollment meetings and promptly return paperwork to the home office
    • Gain knowledge and stay informed of changes regarding PDO services

    The Field Coordinator will project a positive image and will be the face of CDCN with the Consumers and general public. They will serve as the local point of contact for the branch office to utilize for field assignments. 

    • Per Visit Payment: The Coordinator will be paid on a per visit basis as follows:   
      • $90.00 for less than 50 mile RT visit
      • $110.00 for a 50 - 100 mile RT visit   
      • $120.00 for a greater than 100 mile RT visit


    This payment is all inclusive of the visit requirements including scheduling and assembling enrollment packets, completing the visit, auditing the completeness of the enrollment papers, documenting, and forwarding information to the branch office. CDCN is not providing a guarantee of visit volume in this agreement.

    •  Administrative/Training Hours: The Coordinator will be reimbursed at a rate of $12.00 per hour for time spent receiving training.
    • Transportation & Vehicle Insurance: Coordinator will need to have reliable transportation for business travel in connection with work for CDCN (Coordinator agrees to maintain liability insurance with a minimum coverage of $300,000.00 per occurrence)



    • Must be able to successfully pass level 2 background check
    • Must have reliable transportation, car insurance and valid driver license for travel in the surrounding
    • Bilingual English/Spanish required
    • Travel required
