    Consumer Direct Care Network is seeking a people-oriented, customer-focused individual to join our team located in our Albuquerque (or Rio Rancho), New Mexico location. The Program Coordinator will be responsible for assisting in the maintenance and coordination of face-to-face intake, referral, and discharge systems to assure client’s needs are appropriately met. This position will coordinate these activities the Albuquerque location and surrounding areas. The Program Coordinator is responsible for helping to provide a cohesive office environment as well as support continuous quality and improvement.

    • Answer incoming telephone calls, determine purpose of callers, and forward calls to appropriate personnel or department

    • Record and deliver messages when appropriate personnel are unavailable

    • Answer questions about organization and provide callers with address, directions, and other information

    • Process consumer charts including new intakes, annual reviews, transferred cases, and shared cases

    • Receive, screen, respond, and complete intake information from inquiries for agency services

    • Responsible for forwarding necessary information to timesheet auditor and the billing/accounting department

    • Act as a troubleshooter for inquiry calls or drop in visits regarding general program information or for consumer complaints

    • Complete assessments for intakes, annual reviews, and change of needs and provides advocacy services in response to referrals and requests from the community

    • Process employee applications and route payroll information in a timely manner - processing to include completeness of application, follow-up with incomplete application information, logging demographic information in consumer file, complete new hire report, background checks, fingerprinting and Federal checks in a timely manner

    • Create and maintain health and personnel files

    • Assist in maintaining and replenishing state required forms as needed

    • Attend educational meetings, training sessions and other meetings as needed or required by program

    • Welcome on-site visitors, determine nature of business, and announce visitors to appropriate personnel

    • Travel in the surrounding area required



    • Bachelor’s degree from four-year college or university preferred

    • 2 to 4 years' related experience and/or related experience and/or training; or equivalent combination of education and experience

    • Bilingual in Spanish preferred but not required

    • Must be able to successfully pass a back ground screening

    • Must have reliable vehicle
