    POSITION OVERVIEW

     

    Consumer Direct Care Network is seeking a self-starting, energetic and people-oriented Direct Care Coordinator to work in a dynamic and fast paced environment located in our Tucson, Arizona office. The Direct Care Coordinator will oversee all scheduling of the direct care worker with members for CDCN, which will include daily operations with data entry.

     

    Successful candidates will possess strong teamwork skills, provide exceptional customer service, and support continuous improvement of day-to-day operations.

    WHAT YOU WILL DO

     

    • Manage all scheduling activities, matching client schedules with available caregivers
    • Work directly with Regional Coordinators in managing/staffing of new referrals, enforcing practical schedules that accommodate both members and caregivers
    • Effectively collaborate with Program Coordinators and other administrative staff to ensure proper communication of scheduling tasks and calls
    • Manage incoming and outgoing telephone calls, responding to basic questions and/or forwarding to appropriate personnel when required
    • Attend meetings and trainings as required
    • Oversee processing of employee and caregiver applications, assuring completeness of application, fingerprinting, background checks, CPR/FA, TB Test to include scheduling of CPR/FA and DCW training classes
    • Assist with administrative duties including sorting mail, assembling care coordination packets and other duties as required
    • Maintain prospect list of on call caregivers for weekend on call staff
    • All other duties as assigned

     

    QUALIFICATIONS

     

     

    • AA Degree or higher preferred or two years related field experience/education
    • Bilingual English/Spanish preferred
    • Must have reliable transportation and valid driver's license
    • Must have initiative and proven reliability.
    • Demonstrated computer skills to include Microsoft Office products
    • Excellent written and oral skills
    • Ability to frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
