Consumer Direct Care Network is seeking a people-oriented Case Manager/Support Broker to work in a dynamic yet fun and collaborative environment located in our Farmington, NM office. In this important role you will assist our clients with developing support plans through New Mexico’s self-directed programs for authorized services and support the ongoing activities of those plans. Successful candidates will emphasizes the coordination of care for individuals with disabilities and promote prompt, respectful and professional service possible to our clients.

• Provide a positive, professional, empathetic and client-focused manner
• Multi-task
• Good verbal and written communication skills that includes the ability to network and build collaborative relationships
• Speaking effectively with clients and employees and writing routine reports/correspondence
• Team player- ability to work in teams, are open and receptive to coaching and feedback
• High attention to detail




  • Bachelor’s degree in a related field
  • 6 years’ experience serving individuals with disabilities OR combination of education and experience
  • Must possess computer proficiency specifically with Outlook and Excel
  • Must maintain reliable transportation and valid insurance for some in-state travel
  • Must be able to successfully pass a background screening
