POSITION OVERVIEW

Consumer Direct Care Network is seeking a people-oriented Case Manager/Support Broker to work in a dynamic yet fun and collaborative environment. In this important role you will assist individuals with a variety of tasks related to managing their self-directed services. Successful candidates will emphasize their ability to support individuals with disabilities and the elderly, allowing them to remain in their home by promoting prompt, respectful and professional service.

WHAT YOU WILL DO

  • Provide services in a positive, professional, empathetic and client-focused manner
  • Conduct home visits for program enrollment and ongoing monitoring
  • Assist members with monitoring their self-directed budget
  • Complete appropriate and detailed documentation and correspondence for members, including all required forms.
  • Assist members with recruiting, screening, and interviewing potential caregivers
  • Review of caregiver timesheets
  • Effective verbal communication
  • Ensure a positive, collaborative and cohesive team environment
  • Attend educational meetings, training sessions, and other meetings as needed or required by the program.
  • Ensure quality customer service

 

QUALIFICATIONS

  • Must meet one of the following educational criteria:
    • Bachelor’s degree with two years’ experience working with individuals with disability or the elderly
  • Case management experience a plus
  • Effective verbal and written communication skills
  • Computer proficient specifically with Outlook and Excel
  • Ability to travel throughout Wisconsin (if using own vehicle must have valid insurance)
  • Must be able to successfully pass a background screening
