Consumer Direct Care Network is seeking a people-oriented, customer-focused individual to join our team located in our Sunrise location. The Program Coordinator position is now available with the option to reside anywhere within the state. Coordinator will be responsible for assisting in maintaining and coordinating face-to-face intakes, referrals, and discharge systems to assure client’s needs are appropriately met for not only the Sunrise location but surrounding areas as well. As a Program Coordinator it will be your responsibility to provide cohesiveness to the office environment, as well as support continuous quality and improvement of the office as related to management of the day-to-day operation. Travel in the surrounding area required.

  • Answer incoming telephone calls, determine purpose of callers, and forward calls to appropriate personnel or department
  • Responsible for ensuring compliance with contractual requirements, HIPAA and state regulations
  • Maintain caseloads, track compliance, keep up to date on regulations related to each Participant Directed Option recipient
  • Assist Regional Coordinator with training, answering questions related to daily operations, processes and procedures, new developments, best practices, etc.
  • Receive, screen, respond, and complete intake information from inquiries for agency services
  • Track and monitor requirements by utilizing spreadsheets and web-based applications
  • Prepare and process Participant referrals for new intakes and transferred cases, this includes face to face meetings with Participants
  • Notify MCO (Managed Care Organization) of any changes in Participant medical condition, newly identified needs, plan of care and suspected abuse/exploitation of Participant
  • Assist in preparing for Managed Care Organization audits
  • Responsible for forwarding necessary information to timesheet auditor and the billing/accounting department
  • Inform Regional Coordinator of any documented activity in the field that might be viewed by Medicaid or Consumer Direct as fraudulent in nature and/or against program mandates and guidelines
  • Act as a troubleshooter for inquiry calls or drop in visits regarding general program information or for consumer complaints
  • Process employee applications and route payroll information in a timely manner
  • Complete new hire report, background checks, fingerprinting and Federal checks in a timely manner
  • Assist in maintaining and replenishing state required forms as needed
  • Attend educational meetings, training sessions and other meetings as needed or required by program
  • Write Standard Operating Procedures
  • Provide training to staff
  • Monitor and report Fraud, Waste and Abuse (FWA)
  • Some in-state travel required





  • Bachelor’s degree from a four-year college or university preferred
  • Proficient and working knowledge of Microsoft Office
  • Two to four years related experience in home health care or combination of education and experience
  • Experience in the field of healthcare highly desirable
  • Familiarity with commonly used medical terminology preferred
  • Bilingual in English/Spanish with oral and written communications required
  • Experience in writing Standard Operating Procedures and/or providing training to staff is preferred
  • Must be able to successfully pass a level II AHCA screening 
  • Must have an unrestricted driver's license with a reliable and insured vehicle
