Consumer Direct Care Network, a company dedicated to providing the highest quality of in-home health care services, is eager to fill a Registered Nurse role in Allen, TX.  The RN is responsible for ensuring that patient care is coordinated and managed accordingly and that services are delegated appropriately by clinical personnel. The RN will also be responsible for the training of new Direct Care Staff.

  • Understand Medicare Conditions of Participations for home health and ensure compliance of home health program
  • Delegate and supervisor Direct Care Staff
  • Train Direct Support Staff to provide home health services accordingly
  • Collaborate with home health team to address trends that affect the agency’s outcome and process measures
  • Receive case referrals and review patient’s clinical diagnosis, medications, procedures and clinical course in order to determine home care needs
  • Assign appropriate clinicians and therapists to case, and provide support in establishing and developing plan of care
  • Must be available during operating hours to assist clinicians and therapists as appropriate
  • Conduct quarterly record reviews and communicate findings and recommendations to Clinical Director and appropriate personnel
  • Assist in the screening, interviewing and orientation process of new personnel
  • Assist State Director with the planning and implementation of in-service and continuing education programs
  • Assist in the formulation and implementation of policies and procedures, as well as strategic goals and objectives
  • Lead biweekly case conference meetings with home health team members and staff meetings as needed
  • Participate in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system



  • Registered Nurse with current licensure to practice in the state
  • Graduate of an accredited school of nursing. Bachelor’s degree preferred
  • 1yr supervisory experience preferred
  • Experience in health care setting, preferably home health
  • Knowledge in Medicare home health Outcome and Assessment Information Set (OASIS) preferred
  • Demonstrate excellent observation, verbal and written communication skills
  • Maintain current CPR certification
