Location: Based in Lansing, serving enter state with focus on Southern Michigan                   

 

 

POSITION OVERVIEW

Consumer Direct Care Network is seeking a people-oriented, customer focused Program Coordinator to join our Michigan team where we support people with disabilities and the elderly. The Program Coordinator is based in the Lansing area and will be responsible for assisting in maintaining and coordinating referrals and in-person meetings to assure client’s needs are appropriately met. As a Program Coordinator it will be your responsibility to provide cohesiveness to the organization, as well as support continuous quality, growth and improvement to the company as related to the management of day-to-day operations. The Program Coordinator will be a “telecommuter” and work from their home office.

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:

Act as a liaison between clients, employees and Consumer Direct


Coordinate, schedule and complete meetings for in person intake meetings for the Self Directed Program.


Route all paperwork, including payroll information, in a timely manner for processing.


Abide by and maintain all confidentiality requirements in communications and information regarding clients, employees and business practices.


Assist in maintaining compliance for all program, state and federal rules and forms.


Attend educational meetings, training sessions and other meetings as needed or required.


Represent the company at various community meetings, trainings, conferences and other educational or marketing opportunities.

 

QUALIFICATIONS

Bachelor’s degree from four-year college or university; or two to four years related experience and/or related experience and/or training; or equivalent combination of education and experience.


Must be able to successfully pass a background check.


Must have reliable transportation, car insurance and valid driver license for travel.


Must be very reliable, detail oriented and have exceptional abilities to work well independently in a remote team environment.


Must possess an ability to network, capture and maintain positive and ongoing professional relationships with internal and external teams.


Preferred prior experience in Medicaid and Medicare services in the state of Michigan but not required.
